Aetna, Aetna Better Health, Amerihealth, Blue Cross and Blue Shield, Bravo, Coventry, Coventry Cares, Gateway, Highmark, Humana, Personal Choice, Keystone, Keystone Mercy, Medicare, Horizon, Horizon Workers Compensation Insurance, Horizon Casualty, Cigna, PHCS, Multiplan, Todays Option, Tricare and United Community Health (south only). If your insurance provider is not on this list, please call us to confirm your coverage.

Payment is due at the time of service. We accept the following forms of payment: cash, personal check ( with valid ID ), and credit card ( Visa, Mastercard and American Express ).
